ORDER FORM INSTRUCTIONS:

Use the Order Form below to indicate the number of plant markers you wish to order, the sizes, color of Corian® (white or sandstone), and height of stakes. Your request will be answered promptly via email or by telephone to discuss other pertinent details of your order, and to total prices once that is decided. You will also be given instructions on how to type a plant name list. It will be most expedient to discuss the details of your order by telephone, so please include your home and work telephone numbers.

Please use the COMMENTS section below if you have questions, or need custom size Plant Markers.   Use "SUBMIT" button to send your order.  Marion will call you at a mutually convenient time. If you prefer not to give your phone number via this form, so indicate in "comments" section below, and CES will be happy to supply our phone number. We are in Atlanta, Georgia.

more than the three lines allowed for the Large size Plant Marker

(4 3/4" x 2 1/4"), you may use this form. If your Memorial Marker will require

more than 3 lines, please use our Order Form for "Corian Plaques".

comments you would like to make, or if you wish to order custom size plant markers.  You may submit your list of plant names/memorial tree markers names, etc., via fax , email, or a Microsoft Word .doc file. Once we speak with you by telephone, we will direct you on how to submit your list. We will also discuss payment options at that time.

NOTE: When you  hit the "submit" button below you will be shown a confirmation page and your order form should be sent via e-mail to Custom Engraving Services. Please copy (or print) the confirmation page and if you do not hear back from CES within 48 hours, please e-mail us at auntm@mindspring.com to let us know you filled out the order form and sent it to us.
